Abstract
Lymphatic tissue is present throughout our whole body and each lymphoid tissue has its own function. Cells present in lymphatic tissue are B-cells, T-cells, and type of WBC (White blood cell).
Lymphatic tissues are present in
- Lymph node:- It is the small structure present in our body found on the neck, axilla, etc and it helps in removing toxins from the body.
- Thymus:- It is the organ found on the upper part of the breast bone. It helps in the development of T-cells
- Spleen:- Which helps in making lymphocytes and building immunity to fight infection.
- Adenoid and Tonsils:- It Is collectively known as lymphatic tissue present in our throat. Protects us from inhaling germs in our bodies.
- Bone marrow:- It consists of RBC, WBC and platelets.
All these Lymphatic tissues help in boosting immunity and fighting any kind of infection. But when this lymphatic tissue does not work properly and is infected it leads the cells to divide abnormally causing them to form a tumor.
What is Castleman Tumor?
Castleman tumor is a rare tumor of the lymph node and its tissues. It is noncancerous and is not dangerous. It is known as a lymphoproliferative disorder which means the cells grow abnormally present in the lymphatic system similar to lymphoma (cancer of lymphatic tissue). But one thing to note that cases of Castle tumor with malignancy have not been noted.
There are two types of Castleman Tumor
- Unicentric Tumor
- Multicentric Tumor
Unicentric Tumor:- It is one of the commonest types of Castle tumor. This tumor does not have the tendency to spread from one place to another and is commonly found in the chest or abdomen. If the lymph node is enlarged it might press the windpipe.
Multicentric Tumor:-It affects more than one group of Lymphatic tissue. The tumor can be found or detected in more than one place on the body. People with HIV or AIDS have a high tendency to develop Multicentric tumors.
Other micro types tumor includes
- Plasma cell tumor
- Hyaline vascular tumor
- Mixed type of both
Symptoms showed in Castleman Tumor
The symptoms may vary depending on the location of the tumor. The most commonly presented symptoms are:-
- The presence of an enlarged lymph node can be seen on the neck, abdomen, and swollen lump can be felt under the skin.
- If the tumor is present in the chest then it's most likely to press the windpipe causing the patient to breathe difficulty and fullness in the chest.
- If the tumor is present in the stomach then the patient may suffer from flatulence, pain in the abdomen, etc.
- Infection
- Fever
- Loss of weight
- Weakness/Fatigue
- Night sweats.
- Low RBC count (Anemia)
- Hypergammaglobulinemia (High levels of antibiotics) is also noted in patients with Castleman tumor.
- Other symptoms are shown in multicentric tumors include:- Multiple tumors on chest, groin, axilla, and side of the neck and internal organs like the liver and spleen are also affected.
Risk factors to develop Castleman tumor
- People who smoke are most likely to develop tumors.
- People who have a family history with tumors have chances to suffer from Castleman tumors.
- HIV and AIDS patients because the weak immunity has the tendency to catch the infection and develop the Tumor.
Causes for Castleman Tumor
People who have Castleman Tumor have abnormally high levels of blood in response to immunity which can cause tumors. For example, in multicentric cases, the body produces too much protein called Interleukin-6 to regulate immune function. High levels can cause the lymphocyte to grow and lead to tumors.
One of the causes is an infection caused by Human herpesvirus (HHV8) also known as Kaposi sarcoma herpesvirus. The virus can lead to abnormal growth of the cells to form the Castleman tumor.
Test for Castleman Tumor
Medical history:- To know about any illness suffered in the past or habit of smoking or drinking.
Family history to know the presence of tumor in the family.
- Blood test:- It includes CBC, ESR, IL6, C-reactive protein which helps to know the infection or any other abnormality in the blood which causes the tumor.
- CT needle biopsy:- A small sample from the node is taken and is looked at under a microscope to confirm the diagnosis.
- USG:- To look for a tumor in the liver, spleen or any other internal organ.
- PET scan:- To confirm whether it is benign or malignant growth. It is a multicentric or unicentric castle tumor
- Lymph node biopsy:- The tissue is taken from affected node to confirm the diagnosis
