Abstract

Testicular microlithiasis is an uncommon condition.  It is diagnosed on a follow up ultrasound of the testes or scrotum. This condition arises when small clusters of calcium get deposited around both of the testis. In almost 90% of cases the diagnosis was made accidently. The tests are performed related to some other condition and the result comes out as testicular microlithiasis. Many healthy men who do not have any symptoms are often diagnosed with this condition during usual follow up. The condition is commonly not associated with cancer and most of the time remains asymptomatic. In this article we will discuss testicular microlithiasis in detail along with its ayurvedic management.

Introduction

Male testis are two egg shaped glands which are present below the penile region. These are accumulated inside a subcutaneous pouch called scrotum. The testes are slightly spongy but after calcification they feel a little hard. Testicular microlithiasis is seen in around 0.6% of patients that undergo scrotal ultrasound. In 50% of the cases men with germ cell tumor (which further can turn into malignancy) are diagnosed with Testicular microlithiasis. Though any common connection between these two is difficult to make. The condition represents itself as accumulation of tiny calcified particles which surround both of the testis. It generally affects seminiferous tubules. Testicular microlithiasis is very much potent in leading the 20% male population towards subfertility.

Factors Responsible of testicular microlithiasis

Conditions like this can develop due to very usual causes. Specifically, no exact cause is known for the occurrence of microlithiasis. Some relatables that are mentioned include, cryptorchidism, infertility, mumps and malignant germ cell cancer. Generally its direct relation to cancer has been a topic of debate for many years and still no clear evidence is not present.

Symptoms and signs for testicular microlithiasis

Testicular microlithiasis is an asymptomatic condition and generally no signs are present. Patients when diagnosed accidently, present with the following symptoms.

The severity of symptoms is not very high

  • Presence of lump, which does not show any sign of pain but may vary in size with time.
  • The swelling in the scrotal region will be of unexplained nature.
  • Usually testicles have a spongy appearance but in this case they will feel a little hard on touch or palpation.   
  • Enlargement of lymph nodes in abdominal or neck area.
  • Lower abdomen will bear a heavy feeling.
  • Pricking pain in the lower abdomen.
  • Scrotal region can be tender along with continued pain.

Diagnosing testicular microlithiasis clinically

The easiest way to diagnose it is to ask the patient to have an Ultrasonography test. The sonography report will show the accumulation of micro calcifications surrounding the testes. Mostly below mentioned outcomes are noticed in ultrasound of testes, in case patient has testicular microlithiasis:

Tiny calcifications seen in or around the testes. In addition classical types of testicular microlithiasis have more than five microcalcifications that are generally diagnosed after sonography.Moreover, the patient has a limited type of testicular microlithiasis then less than five microcalcifications are seen in the ultrasonography reports.

According to ayurvedic perspective testicular microlithiasis is the result of shukra dhatu dushti or vitiation. It can also be understood as the shukra dhatu srotas dhusti in presence of kapha that results in the small calcifications in the testis or around the testis. This condition will be managed accordingly by pacifying shukra dhatu and kapha. The symptoms may include toda (pricking pain) in the testis, sathulta (heaviness) and granthi (firm and immovable mass).

Treatment

Modern medicine has no specific treatment for testicular microlithiasis. Modern practitioners advise patients to look after their symptoms and signs. After the onset of any major changes, specific action is taken.
