Abstract

Hyperaldosteronism is an endocrine disorder. Endocrine disorders are those which are related to impaired secretion of endocrine glands. Endocrine glands include Hypothalamus, Pituitary, Thyroid, parathyroid, Adrenal gland etc which secretes hormones. Hormones helps in the process of regulation of body and each hormone have different function. Common endocrine disorders are diabetes mellitus, acromegaly, Addison’s disease, Cushing syndrome, Grave’s disease etc. Here we are going to discuss in detail about Hyperaldosteronism.

What is Hyperaldosteronism?

It is an endocrine disorder which involves one or both adrenal glands to secrete too much of aldosterone. Due to its excessive secretion the potassium level decreases and sodium level increases in the body and the patient is having high blood pressure, water retention in the body etc.

Ayurvedic aspect of Hyperaldosteronism

As per Ayurveda, diseases are caused by vitiation of the tridosha’s i.e (vata, pitta, kapha). Impaired function of Aldosterone hormone has two main symptoms i.e. hypertension and hypokalemia. Hypertension is due to vitiation of Vata dosha (Vayan vata). Vata dosha vitiation is due to vataja nidan or other dhatus and doshas. The pathway of Vata is obstructed by other dhatus and doshas. In this we can consider the concept of avarana due to which hormonal secretions are impaired and atherosclerotic changes occurs in the body. The other symptom includes decreased potassium level which is also due to vitiation of vata dosha. The meda dhatu, pitta and kapha dosha obstruct the vata dosha pathway which results in impairment of aldosterone hormone.

What are the symptoms of Hyperaldosteronism?

Due to hyperactivity of aldosterone hormone, it results in following symptoms:-

• Shortness of Breath
• Hypokalemia (low potassium level )
• Muscle Cramps
• Excessive Thirst

What are the types of Hyperaldosteronism?

It is of two types.

1. Primary hyperaldosteronism (Conn’s Disease)
2. Secondary hyperaldosteronism
3. Both the types have same symptoms but the causes are different.

What are the causes of Hyperaldosteronism?

According to its types, Hyperaldosteronism has different causes:-

1. Primary Hyperaldosteronism (Conn‘s Disease)

The main cause of primary hyperaldosteronism is adrenal hyperplasia and adrenal adenoma. It causes aldosterone producing cells of adrenal cortex to release excessive aldosterone resulting in high blood pressure and decreased potassium level in the body. This occurs due to pathology within the adrenal gland.

2. Secondary Hyperaldosteronism

It is due to over activity of Renin-angiotensin-aldosterone- system (RAAS). It includes blockage or narrowing of renal artery, Chronic liver failure, excessive use of diuretic medications, Heart failure etc. This occurs due to pathology outside the adrenal gland.

How to Diagnose Hyperaldosteronism?

The diagnosis may include the following investigations:-

1. Blood tests :- Blood tests shows renin and aldosterone levels in the body. If the hormonal (aldosterone) level increased and renin level decreased that indicates Hyperaldosteronism.
2. Computed Tomography (CT Scan):- This is the other test which helps in finding any tumor , congenital abnormality, hyperplasia etc.
3. Magnetic Resonance Imaging (MRI ABDOMEN):- The other way of finding adrenal gland abnormality i.e. hyperplasia, congenital tumor, adenoma etc. Due to this, it’s easy to find the cause of hyperaldosteronism .

Other Tests May Include

1. Captopril challenge test
2. Saline infusion test
3. Salt loading test
4. Fludrocortisone suppression test
5. Adrenal vein sample

What are the complications of Hyperaldosteronism?

The complications may occur when the secretion of aldosterone hormones is much increased. These are :

1. Heart attack
2. Kidney failure
3. Stroke
4. Injury to heart tissues
5. Enlargement of heart
